Your Health Journey Starts Here
Your Functional Medicine Journey

Functional medicine seeks to identify the root causes behind why your body may not be functioning optimally. Your body in fact has send you myriads of messages, yet most of us have not learned how to listen to our bodies. That is where we start! As a functional medicine doctor I am a health detective and I need your help to gather detailed information.

We’ve designed detailed intake forms for you to complete before your visit with us.
We understand it may take some time, but this is your opportunity to reflect on:
-
Your past medical history
-
Your current health concerns
-
The reasons that led you to seek our support
Having all of this information documented in advance is incredibly helpful.
When you arrive, we’ll review it together during your medical or nutrition consultation.
We’ve found that many clients struggle to recall everything when sitting in front of us—so this process sets you up for success!

Why Your GP Needs to Know You’re Seeing a Functional Medicine Practitioner
As part of your healthcare journey, it’s important to let your GP know that you’re seeking additional support through functional medicine. This isn’t about replacing your GP—it’s about creating a more complete and collaborative picture of your health.
Functional medicine looks deeper into the root causes of illness, which means understanding your full medical history is essential. With your permission, I’ll write to your GP to request relevant information such as past test results, diagnoses, and current medications. This helps prevent unnecessary duplication and ensures that any treatment we explore together complements your existing care.
Open communication between practitioners is in your best interest.
It allows us to work together more effectively, keeping your safety and overall well-being at the center of everything we do. If you’re ready, you can download the letter to share with your GP.

Roughly 70–85% of functional medicine cases can be significantly clarified or resolved through the combined use of gut testing, genetic testing, and hormone testing — provided these tests are interpreted holistically and in the context of a patient’s symptoms, lifestyle, and medical history.
Personally, I consider this trio as a baseline for all functional medicine patients, which is reflected in the assessment packages I offer.

Once we have collected the necessary information, we can start assembling your initial treatment plan, which likely includes:
A tailored nutrition plan
Treatment options addressing the root causes and functional imbalances identified through testing
Clinically relevant supportive nutrients
Significant lifestyle changes designed to complement your personalized treatment plan and support your health goals and challenges
